  • Helping seniors stay independent by assisting with everyday essentials:

    • Light tidying & organization

    • Online grocery shopping

    • Mail sorting & bill reminders

    • Appointment asistance

    • Medication reminders (non-dispensing)

    *Note* In person check-Ins & task Support once/month

  • Thoughtful, 15minute scheduled phone calls that focus on:

    • Friendly conversation & emotional support

    • Wellness updates emailed to family

    • Task reminders

    • Pick up prescriptions

    • Communicate any issues or delays with family/caregivers

    Note: meds need to be located at one pharmacy

  • Keeping loved ones in the loop:

    • Weekly email recaps after visits or calls

    • Notable updates on well-being or concerns

    • Suggestions for small ways to support from afar

  • In-home visit, checklist + Email updates - $125-200 (depending on size of home)

    Checklist includes:

  • 15–20 min call + 1 email update to family

  • Empowering seniors to stay connected and confident:

    • Setting up and troubleshooting smartphones, tablets, or smart TVs

    • Creating “tech how-to” cheat sheets-$55/hour

Lifestyle Wellness Plan $399/Month

  • Helping seniors stay independent by assisting with everyday essentials:

    • Light tidying & organization

    • Online grocery shopping

    • Mail sorting & bill reminders

    • Appointment planning

    • Medication reminders (non-dispensing)

    Limitations in person checks twice/month

  • Supporting physical activity and personal goals:

    • Guided neighborhood walks or chair yoga/stretching

    • Encouraging hydration, nutrition, and healthy habits

  • Empowering seniors to stay connected and confident:

    • Setting up and troubleshooting smartphones, tablets, or smart TVs

    • Teaching video calling (Zoom/FaceTime)

    • Helping manage email, social media, or photo albums

    • Creating “tech how-to” cheat sheets

  • Promoting independence with peace of mind:

    • Visual assessment of trip hazards, lighting, and accessibility

    • Suggestions for aging-in-place improvements

    • Seasonal safety prep (winter rugs, summer airflow, etc.)

Circle Of Care Plan $799/Month

  • Helping seniors stay independent by assisting with everyday essentials:

    • Light tidying & organization

    • Online grocery shopping

    • Mail sorting & bill reminders

    • Appointment planning

    • Medication reminders (non-dispensing)

    Limitations in person checks 30min x4 per month

  • Comprehensive insight into your loved one’s overall wellbeing. Scored as stated by client

    • Track mood, sleep, appetite, mobility & hydration

    • Energy levels/steps (if they have a measuring device such as apple watch)

    • Appointment attendance.

    • Cleanliness of environment

    • Spot early signs of behavior or routine changes

    • # of times in a week spent doing hobbies.

    • Send weekly wellness summaries to family

    Note: Data will need to be collected over 1-2 months to show a trend.

    Data is collected based on a 0-5 scale or measuring device as appropriate.

    Graph will be sent to family via email.

  • When your loved one needs support during a hospital stay

    • RN-guided advocacy (questions, note-taking, communication with staff)

    • Emotional support and clarity during medical interactions

    • Follow-up summaries for family members

    Note: one hospital visit update per week. Visit will be after the 2nd day of admission.

    • Pickup and return (from home or facility)

    • Door-to-door wheelchair or mobility assistance

    • Travel to a nearby park or greenway

    • 20–30 min outdoor walk or push, paced to comfort

    • Light social engagement, rest breaks, water/snacks

    • Return home and safe transfer inside

    Note: Must be option for a suitable safe trail with in 15 minutes of your home location.

  • Helping families navigate care transitions with clarity and compassion:

    $55 per hour (fee waived with accepting facility)

    • Personalized support finding the right senior living or care community

    • Coordination with local resources and trusted partners

    • Guidance on questions to ask and what to look for

    • Emotional and logistical support throughout the transition process

    *Fee waived if successfully placed

  • When your loved one needs support during a hospital stay
